    After reflecting on #HLTH2023, here’s 5 takeaways that crystallized as top of mind💎cultural shift from Dr knows best ➡️ patient knows best ➡️ people enhanced with the power of AI know best 🤖🤝💎 success of hospital at home & RPM requires changing the lens from patient-centered ➡️ person-centered ➡️ life-focused care ✅💎 reimbursem*nt is the dominant primary complaint & barrier to connecting patients to necessary care of the future; cue out-of-the-📦 reimbursem*nt reimagining ✨ 🚨WARNING: do not limit access to information, tools, technology based on assumptions of what patients can afford. All individuals have a right to being presented with ALL treatment options, digital health included, allowing patients to determine what works best for them. These decisions need to be supported to address financial toxicity. 💎 initiatives comprised of multi-stakeholder, private, public, PATIENT partnerships will be winning drivers of innovation adoption, implementation, and meaningful change 💎 companies, innovators, providers, & health systems are not doing enough to help support patients in self-advocating for inclusion of the latest tools & technologies in their care. If I have a problem with medical records, I go to the privacy officer. If I have a crummy experience, I go see the chief experience officer.     At Apex, we're all about using #dataforgood. Putting the right information into the right hands at the right time, for the benefit of those most impacted. New York’s school-based health centers (SBHCs) lacked statewide data that would allow them to identify gaps in care, document outcomes, improve operations, advocate for their work, or operate in an evidence-based world. In 2018 the New York School-Based Health Foundation and the New York School-Based Health Alliance set out to address this critical need, obtaining the support of several private foundations and joining with Apex Evaluation to build a New York SBHC Statewide Data Hub.After a needs assessment and feasibility analysis, New York launched a pilot program. Multiple sponsoring organizations now submit visit data to our Hub, and Apex helps them prep and submit required state quarterly reports.The result? Hours of manual work on behalf of providers now reinvested back into the care of students.This report on New York SBHCs is a first. It shows data from the 2020-2021 school year—the first full year of data collection—from the first five participating SBHC sponsoring organizations. These five organizations represent 20% of all SBHCs in New York, a number that increased to 33% in year two. It's just one of the many powerful uses of the Data Hub to strengthen New York SBHCs, and our goal is to serve all sites in the state.We're just getting started, and can't wait for what's next. #schoolhealth #newyork #bigapple #studentsmatter #reportingtools #patientcare Sarah M.
